New options for enteral nutrition in patients with severe acute pancreatitis




Severe acute pancreatitis (SAP) requires an adequate nutritional support. Enteral nutrition (EN) should be preferred to total parenteral nutrition in patients with SAP, as it is associated with reduced mortality and complications. However, in clinical practice EN is employed far less frequently than it should. The main obstacle to EN diffusion is that it is considered complicated, as to ensure full pancreatic rest, nutrition tubes should be placed in the jejunum, requiring often troublesome procedures. In the past few years, it has been proposed that EN through nasogastric (NG) tubes may be a simple, safe and equally valid alternative to nasojejunal tubes.


A research article published on August 7, 2010 in the World Journal of Gastroenterology addresses this question. The authors speculated that a pragmatic possibility in real-world clinical practice would be to employ NG feeding whenever tube migration to the jejunum of bedside inserted feeding tubes does not occur spontaneously. They therefore aimed at assessing the rate of spontaneous distal migration of EN tubes in patients with predicted SAP, to identify possible factors associated with it, and to compare the safety and tolerability of EN with an elemental formula in patients who started nutrition with a "proximal", NG or a "distal", naso-intestinal tube, depending on the success of spontaneous tube migration.


This is the first study of its kind observing the outcome of EN in SAP patients in a "real world" clinical setting, with the study protocol driven by the need to have more solid grounds in making clinical decisions about everyday medical care circumstances. Both the proximal and the distal enteral approaches resulted to be feasible, safe and effective in most patients. This issue has a relevant impact on everyday clinical practice as the main limit to EN usage in AP is the technical difficulty in obtaining small bowel access.

Male maturity shaped by early nutrition


Differences between the sexes affected by environment during first six months of life



EVANSTON, Ill. --- It seems the old nature versus nurture debate can't be won. But a new Northwestern University study of men in the Philippines makes a strong case for nurture's role in male to female differences -- suggesting that rapid weight gain in the first six months of life predicts earlier puberty for boys.


Males who experienced rapid growth as babies -- an indication that they were not nutritionally stressed -- also were taller, had more muscle and were stronger, and had higher testosterone levels as young adults. They had sex for the first time at a younger age and were more likely to report having had sex in the past month, resulting in more lifetime sex partners.


The researchers think that testosterone may hold the key to understanding these long-term effects.


"Most people are unaware that male infants in the first six months of life produce testosterone at approximately the same level as an adult male," said Christopher W. Kuzawa, associate professor of anthropology in the Weinberg College of Arts and Sciences and author of the study. "We looked at weight gain during this particular window of early life development, because testosterone is very high at this age and helps shape the differences between males and females."


The study provides more evidence that genes alone do not shape our fate.


"The environment has a very strong hand in how we turn out," Kuzawa said. "And this study extends that idea to the realm of sex differences and male biology."


The study found men, on average, tend to be taller and more muscular than females, and the magnitude of that difference appears to be the result of nutrition within the first six months of an infant male's life, according to the study.


"There is a perennial question about how important heredity is versus the environment as shapers of who we turn out to be," said Kuzawa. "In the last 20 years, a lot has been learned about a process called developmental plasticity -- how the body responds early in life to things like nutrition and stress. Early experiences can have a permanent effect on how the body develops, and this effect can linger into adulthood. There is a lot of evidence that this can influence risk of diseases like heart attack, diabetes and hypertension -- really important diseases."


Kuzawa and his collaborators applied the same framework in this study and found evidence that male characteristics -- such as height, muscle mass and testosterone levels as opposed to disease characteristics -- also relate back to early life developmental plasticity.


"Another way to look at it is that the differences between the sexes are not hard wired, but are responsive to the environment, and in particular to nutrition," Kuzawa said.


Testosterone has long been known to increase muscle mass and puts a person on a higher growth trajectory to be taller. The Northwestern study suggests that the age of puberty also is influenced by events in the first six months of life.


The study, which was funded by the National Science Foundation and the Wenner Gren Foundation, was conducted among a group of 770 Filipino males aged 20 to 22 who have been followed their entire lives. Since 1983 a team of researchers in the United States and the Philippines (including Kuzawa for about the last 10 years) has been working to understand how early life nutrition influences adult health, such as risk for cardiovascular disease and diabetes.


"Rapid Weight Gain After Birth Predicts Life History and Reproductive Strategy in Filipino Males" was published Sept. 13 in the Proceedings of the National Academy of Sciences. The study's co-authors are Thomas W. McDade, associate professor of anthropology, Northwestern University, Linda S. Adair, University of North Carolina at Chapel Hill, and Nanette Lee, University of San Carlos of the Office of Population Studies in Cebu City, Philippines.

Deadlifts involve lifting heavy weights using primarily your back, arms and a bit of leg strength. The form of deadlifts is hard to maintain - you are to move slowly and avoid all locking positions, so your posterior sticks out a bit when you are doing deadlifts. If not done properly, deadlifts can lead to back pains, so don't do it unless you're sure of yourself, and always use light weights till you understand how to do them completely. This is another gret way to gain weight quickly.

Yankees What Happened To Our Don?







Joe Torre shockingly announced he will retire from the Los Angeles Dodgers, and hands  the reins to successor Don Mattingly today  with two weeks left in the regular season.

It's a move that caught everyone by surprise considering that the Dodgers may want to interview others such as Tim Wallach, but nevertheless Mattingly is the incoming manager while Torre is nothing but a lame duck on the West Coast.

This offseason is promising to be quite the boiling one when it comes to managers and vacant team openings. As for Joe Torre, where will he end up going next season?

You can bet he'll end up with a big market team with a playoff bound roster that is destined for the World Series. Remember he is retiring from the Dodgers, not baseball, so whether he is or isn't we look at Torre's possible destinations this offseason. 






Joe Torre is undoubtedly one of the most successful managers in sports history. Starting as a manager for the Yankees in 1996, Torre went on to manage the team for 12 straight years, bringing them to the playoffs in each year. Also add in four championships.

At the end of the 2007 season, Joe Girardi took over as the Yankees manager, and Torre went on to manage the Dodgers. Three years later, he has announced that he will be stepping down as manager of the Dodgers in 2011, and will hand over the job to Don Mattingly.

Save money on back to school supplies this year to keep yourself on budget and allow you to have extra spending money for extra-curricular activities. Many students are forced to go back to school on a budget and are looking for the best back to school tips this year. 9. Backpacks are a big-ticket item that generally takes a large bite out of your back to school budget. Consider purchasing backpacks and book bags online for additional savings. Many websites offer great savings on backpacks such as eBags and School-Backpacks.com.

10. Make a master list as soon as possible and keep it with you every time you are out shopping. Merchants will advertise special savings on school supplies throughout the summer. As you run up on special savings and discounts, purchase the items and mark them off your list.

After reading through the ferocious uproar for the comments made by President Bush, I thought I will also share my thoughts ! If this comment is targeted at the so-called 'struggling middle class' people of the Indian population, Bush must lend his ears to hear. How much does he know about the eating habits of our middle class society? How much does he know about the middle class people who struggle daily to make the ends meet? It is a pity he does not realize that the driving force of the Indian middle class has helped them to achieve their best and move up the ladder. A parent from a middle class spends the major part of the income in giving the best education to the kids. Focusing only on 'Education' they look at education as their only savior. Thousands of middle class citizens of India has filled most of the intellectual jobs in USA. Every year we suffer brain drain by sending the creamy layer of the Indian middle class brains, who can support their country with their services, to serve in a country where they were treated only as 'aliens'. The words of Bush must be an eye opener to our middle class Indians who go abroad only with a notion of earning money and building up their bank balance. They can tell they don't get a good opportunity here at India. Earlier things were like that. But now things are changing.

People who were struggling with a morsel of food everyday would definitely like to have a lavishing lunch or a dinner when they get the necessary money. It is quite normal. Can't he tell a rich man to bring down his lavish dinner so that the money could be spent to feed thousands of starving poor.

Let me tell a story to understand the role of a middle class man in our society. This is a story o f a king called 'Thiru Sanku'.

Kind Thiru Sanku wanted to go to heaven with his mortal body. He requested his Raja Guru Vashishtar to help him. The sage Vashistar informed the Raja that it is not possible and only after death he can go to heaven. The king went to Sage Viswamithra and requested Viswamithra to help him. Sage Viswamithra with his celestial power sent Thiru sanku to heaven. On seeing King Thiru sanku coming to heaven with mortal body, Lord Indra threw him out in space.

Thrown down into the space King Thiru sanku cried 'Help! Help!' Hearing his voice, Viswamithra looked up and stopped King Thiru sanku in mid space. He created a heaven for the King and named it 'Thiru sanku swarga'.

THIS IS THE CONDITION OF MIDDLE CLASS PEOPLE IN INDIA. They could neither move up and shake hands with the rich people nor come down and sit with the poor people. They form their own circle who neither move up nor go down.

It is clear to see that violence and controversy has continued to rise in sporting events as the years roll by us. In baseball, basketball, hockey, and especially football, these occurrences have happened more frequently than in the past seasons. The question is why? Why do these violent acts continue to happen with the fans in the seats and with the players on the field? Several factors are to be considered as influencing these acts in professional sports. 

Let’s face it; sports are life for many people living on this earth. I know from my experience that when I am watching a game on television or in the stands, the intensity of the action on the field encompasses my body. The thing that many people do not think about is the consequences for their actions and how to hold their emotions inside. With an increase in rivalry meetings between teams, there comes such a strong intensity that fans let their emotions run wild and commit acts that otherwise would not be committed. Sometimes, fans even run onto the field even in the nude and not only distract the players on the field and interrupt the fun of the game, but they end up in jail for the night or longer. Also, with many vendors selling alcoholic beverages in the stadiums or arenas, fans’ views can become alternated and they really do not know what they ended up doing. In one instance, a father and son charged onto a baseball field and beat up on a defenseless umpire who made a “bad” call on a previous play. The consequences of their actions ended up not only putting them in jail together, but canceling the rest of the game making the players and the fans suffer.

A second reason for such violence at sporting events is an ongoing case that continues to appear in the world of sports. The reason is steroids. Steroids are becoming a serious issue in all of the major sports and even in the Olympics. By using these performance enhancing drugs, players not only endanger the lives of themselves and their family, but they give players that extra power allowing them to hit the opponent harder and do things that they otherwise would not do. Such instances often end in season or career-ending injuries for the innocent player receiving the hit. Such as in the case with alcohol in the stands, the steroid user does not even think about the consequences of the hit or abuse he puts onto the opponent. Using steroids often causes unwanted violence and sometimes even hurts their own team and career, along with handing a heavy-weighted fine to the abuser. Not to mention that after officials find out about the steroid use, it jeopardizes the career of the user, which in turn causes unwanted violent acts from fans onto the player. 

The final and possibly the most important factor that goes into violence at sporting events is that sports are taken to serious in society. Sports are enjoyable to watch and can be very relaxing to watch after a stress-filled day. One play, however, can cause turmoil between friends, players, and the fans in the stands. An unwanted interaction occurs because of the human need for competition. From personal experience, I know that when two fans are at differences between two competing teams, violence, though not as serious, often occurs. Violence does not have to be limited to physical abuse, but it can also be considered as yelling and verbal abuse, sometimes ending in a broken friendship. Also, society encourages violence in sports. Often times, people cheer at the result of an opposing player being hurt on the playing field by a player from their team of choice. 

It is clear that something must be done to stop the violence in sports. If sports were taken as they were meant to be, than violent acts would be all-around more enjoyable for the fans and the players. Now that the question of why has been answered, the question still remaining is how long, if ever, before violent acts are non-existent in the world of sports? As a huge sports fan myself, I believe that the process will be ongoing for many years to come. I guess sports are that much influenced by society that the serious issue is receiving little thought in the minds of the respected commissioners of each league. Until a more serious consequence is levied upon the abusers, violence in sporting events will continue to meet the needs of society for years to come.

I admit it! Every year, I go out and purchase the latest NASCAR game presented by EA Sports. And each year, I am usually a little dissatisfied.

This year, once again, I had high hopes, but there are several discrepancies regarding NASCAR 2007 that are a nuisance to me. There are just so many ways in which EA Sports can enrich their NASCAR games, and make them more realistic to their loyal buyers.

Firstly, whatever happened to the leaderboard? Ever since NASCAR Thunder 2004, it has vanished completely, and has yet to return.

During the race, I would like to pause and pull up the leaderboard, just to keep check on how my competition is doing. What if I am running in 15th place, and want to see who's running second or third. I may want to see what position Dale Earnhardt Jr, Jeff Gordon, or Tony Stewart may be in. I would like to know exactly which drivers have fallen out of the race. I may be engaging in a heated points battle, and knowing who is where and in what position is imperative.

Also, whenever someone hits the wall, the damage is incredibly unrealistic. I have watched Dale Earnhardt Jr wreck up in front of me, and hit the wall, flip over about three or four times, and the proceed to win that very same race.

If a car hits the wall, it would be nice if the artificial intelligence would at least weaken their performance for that particular race.

In NASCAR 2005 and NASCAR 2006, you could create a car, and use a wide array of single and double digit numbers. That was one feature that really impressed me. Well, in NASCAR 2007, only three digit numbers are accessible. Has anyone ever seen a three digit car out on the NASCAR circuit? Of course not.

NASCAR 2007 has added a driver attribute rating, which is a very ingenious concept. But it would be even better if we could adjust every driver's attributes. Each year, a driver's performance may change. I know that Elliott Sadler is on the cover, but it has been two years since he has been really competitive, yet NASCAR 2007 has him listed with an A- rating. An adjustable driver rating would be a very trendy feature.

And last but not least- if EA Sports would wait until January or February to release their NASCAR games, the current version would not seem so archaic. Elliott Sadler endorses the product, yet he no longer drivers the M&M's Ford Fusion, which is depicted on the cover. He has replaced Jeremy Mayfield in the 19 Dodge, and of course, the game has Mayfield in the 19 Dodge.

And next year, drivers such as Casey Mears, Brian Vickers, Dale Jarrett, and Jeremy Mayfield will be in different cars. Would it not make more sense to wait until the new year to release the NASCAR game, instead of releasing right smack dab in the middle of the season? I don't see EA Sports releasing MADDEN NFL in November.

Oh yeah, and please include all full-time Nextel Cup drivers. In 2005, Jeremy Mayfield was missing in action, and in 2007, Carl Edwards, Ken Schrader, and Clint Bowyer are no where to be found.

EA Sports needs to realize that the same people who buy their NASCAR games each year are serious gamers. They want the most realistic product available. Please give it to them!
